Summary

Nefazodone has serotonergic activity and inhibits CYP3A4, and tapentadol also has serotonergic and noradrenergic effects. Together they may raise the risk of serotonin syndrome, although tapentadol is mainly cleared by glucuronidation so exposure changes are limited.

What this interaction means in plain words

Nefazodone and Tapentadol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive serotonergic effects from nefazodone's serotonin reuptake inhibition and 5-HT2A antagonism with tapentadol's monoaminergic activity.

What to do

Monitor for agitation, tremor, clonus, hyperthermia and diarrhea, particularly after dose changes. Use the lowest effective doses.
