Summary
Neostigmine is used to reverse non-depolarizing blockade by pancuronium, so the interaction is intended and clinically useful. Reversal may be incomplete or recurarization may occur if given when blockade is deep or the dose is inadequate.
What this interaction means in plain words
Neostigmine and Pancuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Neostigmine belongs to Central Nervous System; Pancuronium belongs to Musculoskeletal.
Why it happens
Neostigmine inhibits acetylcholinesterase, increasing acetylcholine that competitively displaces pancuronium at nicotinic receptors.
What to do
Give neostigmine only after some spontaneous recovery is seen on train-of-four monitoring, usually with an antimuscarinic such as glycopyrrolate. Continue monitoring until full recovery and adequate ventilation is confirmed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
