Summary

Neostigmine is used intentionally to reverse non-depolarizing blockade such as vecuronium by increasing acetylcholine at the junction. This is a pharmacological antagonism and is the basis of its therapeutic use, but inadequate dosing or reversal before spontaneous recovery can leave residual paralysis.

What this interaction means in plain words

Neostigmine and Vecuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Neostigmine belongs to Central Nervous System; Vecuronium belongs to Musculoskeletal.

Evidence level: High

Why it happens

Acetylcholinesterase inhibition increases acetylcholine, which competitively overcomes vecuronium blockade at nicotinic receptors.

What to do

Give neostigmine only after some spontaneous recovery, confirmed by train-of-four monitoring, and with an antimuscarinic such as glycopyrrolate or atropine to prevent bradycardia. Confirm full recovery before extubation.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
