Summary
Tranylcypromine is a nonselective irreversible MAO inhibitor with high serotonin syndrome liability, and 5-HT3 antagonists have also been associated with serotonin syndrome. Together they may increase the risk of severe serotonergic toxicity or hypertensive complications. Patients on MAO inhibitors are particularly vulnerable to additive agents.
What this interaction means in plain words
Netupitant-palonosetron and Tranylcypromine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Netupitant-palonosetron belongs to Gastrointestinal Tract; Tranylcypromine belongs to Central Nervous System.
Why it happens
Additive serotonergic effects from irreversible nonselective MAO inhibition and 5-HT3 receptor antagonism.
What to do
Prefer an antiemetic with no serotonergic activity, such as a different class, in patients taking tranylcypromine. If the combination is unavoidable, monitor closely for serotonin syndrome and stop the drugs if it appears.
