Summary

Nilotinib carries a boxed warning for QT prolongation and is also a CYP2D6 inhibitor, which can increase thioridazine concentrations. Thioridazine's QT effect is concentration dependent, so the combination substantially raises the risk of torsades de pointes and sudden death.

What this interaction means in plain words

Nilotinib and Thioridazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Nilotinib belongs to Oncology; Thioridazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive QT prolongation plus CYP2D6 inhibition by nilotinib reducing thioridazine clearance.

What to do

Do not combine; use an alternative antipsychotic with minimal QT liability.
