Summary
Fluoroquinolones can cause both hypoglycemia and hyperglycemia in patients on glucose-lowering drugs. Combined with repaglinide, which stimulates insulin release, hypoglycemia may occur and can be severe.
What this interaction means in plain words
Ofloxacin and Repaglinide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ofloxacin belongs to Anti Bacterial; Repaglinide belongs to Endocrine.
Why it happens
Fluoroquinolones alter pancreatic beta-cell insulin release, adding to the insulin secretagogue effect of repaglinide.
What to do
Monitor blood glucose closely during and after the antibiotic course, and teach the patient to recognize hypoglycemia symptoms. Adjust the repaglinide dose if glucose control changes.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
