Summary
Torsemide promotes urinary potassium loss while olmesartan reduces aldosterone-mediated potassium excretion, so the net potassium effect is unpredictable. In addition, combining a loop diuretic with an angiotensin receptor blocker can cause excessive blood pressure reduction and reduced kidney function, especially in volume-depleted patients.
What this interaction means in plain words
Olmesartan and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Olmesartan belongs to Cardiovascular; Torsemide belongs to Cardiovascular.
Why it happens
Opposing effects on renal potassium handling plus additive hemodynamic effects from diuresis-related volume depletion and angiotensin II blockade.
What to do
Check serum potassium, creatinine and blood pressure after starting or changing doses. Reduce the diuretic dose or correct volume depletion if hypotension or a rise in creatinine occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Olmesartan with Torsemide?
MediHelp rates the interaction as moderate. Olmesartan and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
How serious is the interaction between Olmesartan and Torsemide?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Check serum potassium, creatinine and blood pressure after starting or changing doses. Reduce the diuretic dose or correct volume depletion if hypotension or a rise in creatinine occurs.
What other medicines interact with Torsemide?
MediHelp also lists interactions of Torsemide with, for example, Amikacin, Amiodarone, Carbenoxolone. The full list is on the Torsemide page.
