Summary

Olodaterol, a long-acting beta2 agonist, can cause tachycardia, hypokalemia and QT prolongation, and perphenazine also has QT-prolonging potential. Together they may increase the risk of ventricular arrhythmias, particularly in patients with other risk factors. The systemic exposure from inhalation is low, but caution remains relevant.

What this interaction means in plain words

Olodaterol inhaled and Perphenazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive QT prolongation, with beta2-agonist-induced hypokalemia and sympathetic stimulation increasing arrhythmic susceptibility.

What to do

Check baseline ECG and potassium and magnesium levels in patients with cardiac risk factors. Correct electrolyte disturbances and monitor for palpitations or syncope.
