Summary

Live vaccinia vaccine can cause progressive vaccinia and serious generalized infection in immunocompromised people. Oxaliplatin-related immunosuppression raises this risk and may also reduce protective immunity.

What this interaction means in plain words

Oxaliplatin and Smallpox vaccine live are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Chemotherapy-induced immunosuppression allows uncontrolled replication of live vaccinia virus and weakens the immune response.

What to do

Avoid live smallpox vaccine during oxaliplatin therapy and for about 3 months afterwards. If exposure risk is high, seek infectious disease or public health advice on alternatives.
