Summary
NSAIDs can blunt the blood pressure lowering effect of ACE inhibitors and reduce renal function, particularly in older, volume-depleted or renally impaired patients. Combined with a diuretic this forms the so-called triple whammy, with a higher risk of acute kidney injury and hyperkalemia.
What this interaction means in plain words
Oxaprozin and Trandolapril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Oxaprozin belongs to Musculoskeletal; Trandolapril belongs to Cardiovascular.
Why it happens
NSAID inhibition of cyclooxygenase reduces vasodilatory renal prostaglandins, opposing ACE inhibitor effects and impairing glomerular perfusion.
What to do
Prefer a non-NSAID analgesic where possible. If both are needed, ensure hydration and monitor blood pressure, serum creatinine and potassium within the first weeks and periodically.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
