Summary

Sorafenib has been reported to inhibit CYP2C8 in vitro, and paclitaxel is cleared mainly by CYP2C8 with a contribution from CYP3A4. Exposure to paclitaxel may rise, which could worsen neutropenia and neuropathy. In clinical combination studies the effect on paclitaxel exposure was modest and variable.

What this interaction means in plain words

Paclitaxel and Sorafenib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Paclitaxel belongs to Oncology; Sorafenib belongs to Oncology.

Evidence level: Low

Why it happens

Sorafenib may inhibit CYP2C8-mediated metabolism of paclitaxel, reducing its clearance.

What to do

Monitor blood counts and signs of peripheral neuropathy when the two are combined. Reduce the paclitaxel dose if severe toxicity occurs.
