Summary

Propantheline is an antimuscarinic and paliperidone has modest anticholinergic activity, so peripheral anticholinergic effects such as dry mouth, constipation and urinary retention may be additive. Reduced gastrointestinal motility may also alter absorption of the extended-release paliperidone formulation.

What this interaction means in plain words

Paliperidone and Propantheline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive antimuscarinic effects and slowed GI transit that may change paliperidone absorption.

What to do

Monitor for anticholinergic adverse effects such as constipation and urinary retention, and for changes in antipsychotic response. Choose alternatives to propantheline if these effects emerge.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
