Summary

Polymyxin B has neuromuscular blocking properties and can enhance and prolong pancuronium paralysis. This may cause delayed recovery, respiratory depression or apnea, and reversal with anticholinesterases may be incomplete.

What this interaction means in plain words

Pancuronium and Polymyxin b can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Pancuronium belongs to Musculoskeletal; Polymyxin b belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Polymyxin B blocks prejunctional acetylcholine release and stabilizes the postsynaptic membrane, adding to nondepolarizing blockade.

What to do

Avoid concurrent use if alternatives exist; otherwise reduce pancuronium dosing, monitor with train-of-four and ensure prolonged ventilatory support is available.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
