Summary

NSAIDs such as parecoxib (via its active metabolite valdecoxib) can reduce the natriuretic and antihypertensive effects of loop diuretics and promote sodium and fluid retention. In volume-depleted patients the combination also raises the risk of acute kidney injury.

What this interaction means in plain words

Parecoxib and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of renal prostaglandin synthesis reduces renal blood flow and diuretic-induced natriuresis.

What to do

Monitor blood pressure, fluid status, weight and renal function, and keep the NSAID course as short as possible. Ensure adequate hydration and adjust the torsemide dose if response is reduced.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
