Summary

Pentobarbital is a barbiturate that depresses the CNS, and perphenazine adds to sedation and may lower the seizure threshold. Together they can cause marked drowsiness, hypotension and respiratory depression. Barbiturates also induce hepatic enzymes and may reduce perphenazine concentrations.

What this interaction means in plain words

Pentobarbital and Perphenazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive CNS depression plus CYP enzyme induction by pentobarbital that may lower perphenazine levels.

What to do

Use the lowest effective doses and monitor sedation, breathing and blood pressure. Watch for reduced antipsychotic effect with prolonged barbiturate use.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
