Summary

Piperacillin and tobramycin act synergistically against many Gram-negative bacteria, including Pseudomonas. In severe renal impairment, piperacillin can inactivate tobramycin in vitro and in the body, which lowers aminoglycoside levels. Nephrotoxicity from tobramycin remains a concern.

What this interaction means in plain words

Piperacillin and Tobramycin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Piperacillin belongs to Anti Bacterial; Tobramycin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Synergistic bacterial killing pharmacodynamically; piperacillin can chemically inactivate tobramycin when the two are mixed or accumulate in renal failure.

What to do

Do not mix in the same syringe or infusion bag. Monitor tobramycin levels and renal function, especially in renal impairment.
