Drug interactions
Piperacillin/Tazobactam + Voriconazole
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive hepatotoxicity and potential QT prolongation risk; piperacillin/tazobactam may cause hypokalemia that can potentiate voriconazole-associated QTc prolongation. No clinically relevant CYP-mediated interaction is expected.
Mechanism of action
Pharmacodynamic: overlapping hepatotoxic potential; voriconazole prolongs cardiac repolarization (QTc), and piperacillin/tazobactam can lower potassium, increasing susceptibility to QT prolongation. Pharmacokinetic interaction via CYP enzymes is not expected.
Clinical management
Obtain baseline and at least weekly hepatic panel during concomitant therapy; discontinue or modify therapy for clinically significant liver injury. Monitor potassium and magnesium at baseline and periodically; correct abnormalities (aim to maintain normal range). Consider baseline and follow-up ECG in patients with additional QT risk factors or high voriconazole exposure; minimize other QT-prolonging agents where possible.
