Summary

NSAIDs such as piroxicam can reduce renal blood flow and glomerular filtration, and tenofovir is itself associated with proximal tubular toxicity. Used together, particularly with high or repeated NSAID doses, they may raise the risk of acute kidney injury and increase tenofovir exposure.

What this interaction means in plain words

Piroxicam and Tenofovir disoproxil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive nephrotoxicity from NSAID-induced renal hemodynamic effects plus tenofovir tubular injury, with possible reduced renal elimination of tenofovir.

What to do

Prefer a non-NSAID analgesic such as paracetamol where feasible. If an NSAID is needed, use the lowest dose for the shortest time and monitor serum creatinine, phosphate and urine protein.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
