Summary
Ponatinib inhibits P-glycoprotein in vitro and may raise exposure to tacrolimus, a P-gp substrate with a narrow therapeutic index. Higher tacrolimus concentrations could cause nephrotoxicity, neurotoxicity and hyperglycemia. The clinical magnitude is not well characterized.
What this interaction means in plain words
Ponatinib and Tacrolimus can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ponatinib belongs to Oncology; Tacrolimus belongs to Miscellaneous.
Why it happens
Ponatinib inhibits P-glycoprotein efflux, potentially reducing tacrolimus efflux and increasing its absorption and exposure.
What to do
Monitor tacrolimus trough concentrations and renal function when ponatinib is started or stopped. Adjust the tacrolimus dose to the target trough.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
