Summary

Corticosteroids given with non-depolarizing neuromuscular blockers, especially over prolonged periods in critically ill patients, are associated with acute myopathy and prolonged weakness. Steroids can also modify the response to the blocker.

What this interaction means in plain words

Prednisone and Rocuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Prednisone belongs to Endocrine; Rocuronium belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive muscle injury and neuromuscular dysfunction; the exact mechanism is not fully defined.

What to do

Use the shortest effective duration of neuromuscular blockade and monitor with train-of-four. Evaluate for prolonged weakness and check creatine kinase if myopathy is suspected.

What to ask your pharmacist
