Summary

Primidone is metabolized to phenobarbital and is itself an enzyme inducer. It can speed warfarin clearance and lower INR, risking thrombosis, and stopping it can lead to INR rises.

What this interaction means in plain words

Primidone and Warfarin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Primidone belongs to Central Nervous System; Warfarin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Hepatic enzyme induction, including CYP2C9, by primidone and its metabolite phenobarbital.

What to do

Check INR frequently after primidone is started, changed or stopped. Adjust warfarin to maintain the target range.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
