Summary
Probenecid reduces the renal tubular secretion of tolmetin and other NSAIDs, raising NSAID plasma concentrations and prolonging their half-life. This can increase the risk of NSAID adverse effects such as gastrointestinal and renal toxicity.
What this interaction means in plain words
Probenecid and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Probenecid belongs to Genitourinary; Tolmetin belongs to Musculoskeletal.
Why it happens
Probenecid inhibits organic anion transporters in the renal tubule, decreasing tolmetin's renal clearance.
What to do
Monitor for NSAID adverse effects such as GI upset, bleeding and fluid retention, and use the lowest effective tolmetin dose. Consider reducing the tolmetin dose if toxicity appears.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
