Summary

Procarbazine has weak monoamine oxidase inhibiting activity, so norepinephrine stores and release are enhanced when an indirect sympathomimetic such as pseudoephedrine is given. This can provoke severe hypertension or a hypertensive crisis.

What this interaction means in plain words

Procarbazine and Pseudoephedrine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Procarbazine belongs to Oncology; Pseudoephedrine belongs to Respiratory.

Evidence level: Moderate

Why it happens

MAO inhibition by procarbazine potentiates the norepinephrine-releasing pressor effect of pseudoephedrine.

What to do

Avoid oral decongestants in patients receiving procarbazine and choose non-pressor symptomatic treatment. If exposure occurs, monitor blood pressure closely and treat hypertension promptly.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
