Summary
Vandetanib causes marked, dose-related QT prolongation and torsades de pointes, and quetiapine can also prolong the QT interval. Combining them raises the risk of serious ventricular arrhythmia, especially with electrolyte disturbances or other risk factors. The vandetanib label advises avoiding other QT-prolonging drugs.
What this interaction means in plain words
Quetiapine and Vandetanib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Quetiapine belongs to Central Nervous System; Vandetanib belongs to Oncology.
Why it happens
Additive inhibition of cardiac repolarization (hERG potassium channel blockade).
What to do
Avoid the combination if possible and choose an antipsychotic with lower QT liability. If unavoidable, correct potassium, magnesium and calcium and monitor ECG regularly.
