Summary

Rifabutin induces CYP3A4, which lowers quinidine concentrations and may lead to loss of antiarrhythmic effect. The induction effect is weaker than with rifampicin but still clinically relevant. Quinidine levels may rise again after rifabutin is stopped.

What this interaction means in plain words

Quinidine and Rifabutin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Quinidine belongs to Anti Parasitic; Rifabutin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Rifabutin induces CYP3A4, increasing the metabolic clearance of quinidine.

What to do

Monitor quinidine plasma concentrations and ECG, and adjust the quinidine dose as needed. Reassess the dose when rifabutin is started or stopped, or consider an alternative antiarrhythmic.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
