Summary
Sparsentan is a dual endothelin and angiotensin II receptor antagonist, so adding the valsartan component of sacubitril-valsartan duplicates angiotensin receptor blockade. This raises the risk of hypotension, syncope, hyperkalemia and deterioration of renal function, including acute kidney injury.
What this interaction means in plain words
Sacubitril-valsartan and Sparsentan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Sacubitril-valsartan belongs to Cardiovascular; Sparsentan belongs to Genitourinary.
Why it happens
Additive pharmacodynamic blockade of the angiotensin II type 1 receptor, with combined effects on blood pressure, potassium and renal hemodynamics.
What to do
Do not combine; sparsentan labeling contraindicates concomitant ARBs, ACE inhibitors and other RAAS agents. Stop one agent before starting the other, using a washout in line with the product labeling.
