Summary

Safinamide with a nonselective MAO inhibitor increases the risk of hypertensive crisis and serotonin syndrome. Safinamide labeling contraindicates use with other MAO inhibitors.

What this interaction means in plain words

Safinamide and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Safinamide belongs to Central Nervous System; Tranylcypromine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive MAO inhibition, including loss of MAO-B selectivity, causes excessive monoamine accumulation.

What to do

Do not co-administer. Wait at least 14 days after stopping safinamide before starting tranylcypromine, and allow the MAOI washout before starting safinamide.
