Summary
Selexipag is converted to its active metabolite by carboxylesterases, and the metabolite is cleared by CYP2C8. Tamoxifen may inhibit CYP2C8, possibly increasing active metabolite exposure and prostacyclin-type adverse effects such as headache, flushing and hypotension.
What this interaction means in plain words
Selexipag and Tamoxifen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Selexipag belongs to Cardiovascular; Tamoxifen belongs to Endocrine.
Why it happens
CYP2C8 inhibition reduces clearance of selexipag's active metabolite.
What to do
Monitor for prostacyclin-related adverse effects and consider once-daily selexipag dosing as recommended for moderate CYP2C8 inhibitors.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
