Summary

Tipranavir, given with ritonavir, inhibits CYP3A4 and may increase sildenafil exposure. This raises the risk of hypotension, syncope, visual changes and priapism.

What this interaction means in plain words

Sildenafil and Tipranavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Sildenafil belongs to Genitourinary; Tipranavir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Ritonavir-boosted tipranavir inhibits CYP3A4, reducing the metabolic clearance of sildenafil, although tipranavir also has some inducing effects.

What to do

Start sildenafil at a low dose (for example 25 mg, not more than once every 48 hours) for erectile dysfunction and monitor blood pressure and adverse effects. Avoid use for pulmonary arterial hypertension unless specifically advised by labeling.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
