Summary

Live yellow fever vaccine can cause serious viscerotropic or neurotropic disease in immunosuppressed patients, and the immune response may be inadequate. Sirolimus therapy is a recognised reason to avoid it.

What this interaction means in plain words

Sirolimus and Yellow fever vaccine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Sirolimus impairs lymphocyte-mediated control of live attenuated virus replication and blunts vaccine response.

What to do

Avoid the vaccine in patients on sirolimus; if travel to endemic areas is unavoidable, seek travel medicine advice and consider a medical exemption letter with mosquito-avoidance measures.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
