Summary

Talquetamab causes cytokine release syndrome, and the released cytokines temporarily suppress CYP enzymes. Theophylline levels may rise and cause toxicity such as arrhythmias, seizures or vomiting, mostly during step-up dosing and for some days afterwards.

What this interaction means in plain words

Talquetamab and Theophylline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Talquetamab belongs to Oncology; Theophylline belongs to Respiratory.

Evidence level: Moderate

Why it happens

Cytokine release (notably IL-6) downregulates CYP1A2 and CYP3A4 activity, reducing theophylline clearance.

What to do

Monitor theophylline concentrations and signs of toxicity during step-up dosing and for about two weeks after the first treatment dose, and during any CRS episode. Reduce the theophylline dose if levels rise.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
