Summary
Talquetamab can cause cytokine release syndrome, and inflammation suppresses CYP1A2, the main enzyme clearing clozapine. Clozapine levels may rise and cause toxicity such as sedation, seizures, hypotension and myocarditis-like symptoms.
What this interaction means in plain words
Clozapine and Talquetamab can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Clozapine belongs to Central Nervous System; Talquetamab belongs to Oncology.
Why it happens
Cytokine-mediated downregulation of CYP1A2 reduces clozapine clearance.
What to do
Monitor for clozapine toxicity during step-up dosing and for about two weeks afterwards, and check clozapine levels with dose reduction if toxicity develops.
What to ask your pharmacist
