Summary
Cytokine release syndrome after talquetamab can transiently suppress CYP enzyme activity, which may raise concentrations of CYP substrates such as clomipramine. Higher clomipramine levels could increase anticholinergic, sedative and cardiac toxicity.
What this interaction means in plain words
Clomipramine and Talquetamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Clomipramine belongs to Central Nervous System; Talquetamab belongs to Oncology.
Why it happens
Cytokine-mediated suppression of CYP enzymes (including CYP1A2) reduces clomipramine metabolism.
What to do
Monitor for clomipramine toxicity, including sedation, anticholinergic effects and arrhythmia, from the start of step-up dosing through about two weeks after the first treatment dose and during any CRS. Reduce the clomipramine dose if toxicity appears.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
