Summary
Talquetamab can trigger cytokine release syndrome, and the raised cytokines may temporarily suppress CYP3A4 activity. Colchicine exposure may rise as a result, and colchicine has a narrow therapeutic index with risk of severe toxicity such as GI effects, myopathy and marrow suppression.
What this interaction means in plain words
Colchicine and Talquetamab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Colchicine belongs to Genitourinary; Talquetamab belongs to Oncology.
Why it happens
Cytokine-mediated downregulation of CYP3A4 during cytokine release syndrome reduces colchicine metabolism.
What to do
Monitor for colchicine toxicity (diarrhea, vomiting, muscle pain or weakness, cytopenias) from step-up dosing until about 14 days after the first treatment dose and during CRS. Reduce the colchicine dose or temporarily interrupt it if signs of toxicity appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
