Summary
Terbinafine is a potent CYP2D6 inhibitor and can raise tolterodine levels. Tolterodine is also cleared via CYP3A4, which partly compensates, but anticholinergic effects and QT effects may still increase.
What this interaction means in plain words
Terbinafine and Tolterodine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Terbinafine belongs to Anti Fungals; Tolterodine belongs to Genitourinary.
Why it happens
Terbinafine inhibits CYP2D6, reducing conversion of tolterodine to its active 5-hydroxymethyl metabolite and increasing parent drug exposure.
What to do
Monitor for anticholinergic effects and consider limiting tolterodine to 1 mg twice daily or less if adverse effects occur.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
