Summary
Combining a loop diuretic with a thiazide-like diuretic produces sequential nephron blockade and a markedly stronger diuresis. This helps in diuretic resistance but raises the risk of hypokalemia, hyponatremia, hypomagnesemia, dehydration and renal impairment.
What this interaction means in plain words
Torsemide and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Synergistic sodium excretion from blocking sodium reabsorption at both the loop of Henle and the distal tubule.
What to do
Start with low doses and monitor electrolytes, renal function, weight and blood pressure within the first week. Supplement potassium and magnesium as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
