Summary
Venlafaxine is converted to O-desmethylvenlafaxine mainly by CYP2D6, with CYP3A4 as a secondary route. Voriconazole may increase venlafaxine exposure, raising the risk of nausea, hypertension and serotonergic effects. Both agents can also prolong the QT interval.
What this interaction means in plain words
Venlafaxine and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Venlafaxine belongs to Central Nervous System; Voriconazole belongs to Anti Fungals.
Why it happens
Voriconazole inhibits CYP3A4 (and weakly CYP2C19/2D6 routes), reducing venlafaxine clearance, with additive QT effects.
What to do
Monitor blood pressure, heart rate and serotonergic or adverse effects, and lower the venlafaxine dose if needed. Obtain an ECG if other QT risk factors are present.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
