Mécanisme d'action
Allosteric, reversible inhibitor selective for cardiac myosin; reduces the number of myosin heads entering power-generating states, lowering systolic and residual diastolic cross-bridge formation, reducing dynamic LVOT obstruction and improving filling pressures.
Posologie adulte habituelle (résumé de la notice)
Symptomatic NYHA class II-III obstructive HCM: starting dose 5 mg orally once daily without regard to food; titrate among 2.5 mg, 5 mg, 10 mg or 15 mg once daily; maximum 15 mg once daily

- Unsafe May cause fetal harm based on animal data; no human data. Effective contraception during treatment and for 4 months after last dose.

- Unknown Presence in milk, effects on infant and milk production are unknown; weigh benefits of breastfeeding against maternal need.

Mises en garde encadrées
- Reduces LVEF and can cause heart failure from systolic dysfunction. Echocardiographic LVEF assessment required before and during treatment. Do not initiate if LVEF <55%; interrupt if LVEF <50% or heart failure symptoms or worsening clinical status. Strong CYP2C19 inhibitors and moderate to strong CYP2C19 or CYP3A4 inducers are contraindicated. Available only through the CAMZYOS REMS Program.

- Concomitant moderate to strong CYP2C19 inducers or moderate to strong CYP3A4 inducers
Précautions particulières
- Heart failure risk higher with serious intercurrent illness or arrhythmia; assess clinical status and LVEF regularly.
- CYP450 interactions may cause heart failure or loss of effectiveness, including with OTC omeprazole, esomeprazole, cimetidine.
- Available only via REMS program; prescribers, patients and pharmacies must be certified/enrolled.
