Résumé
Both drugs prolong the QT interval, and combined use increases the risk of torsades de pointes and sudden cardiac death. European labeling contraindicates domperidone with QT-prolonging drugs. Patients with cardiac disease, electrolyte disturbance or other QT risk factors are at greatest risk.

Azithromycin and Domperidone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Pharmacodynamic: additive QT prolongation through hERG potassium channel blockade. Azithromycin is only a weak CYP3A4 inhibitor, so any pharmacokinetic contribution is small.

Do not co-administer. Choose an antibiotic without QT effect, such as a beta-lactam where appropriate, or an antiemetic or prokinetic without QT risk.
