Résumé

Prolonged pantoprazole use can cause hypomagnesaemia, and low magnesium or potassium increases susceptibility to QT prolongation and torsades de pointes with domperidone. This is an indirect, risk-factor-based interaction and is mainly relevant with long-term PPI use. Patients with cardiac disease, older patients and those taking diuretics are at higher risk.

Ce que signifie cette interaction, en termes simples

Domperidone and Pantoprazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Domperidone belongs to Gastrointestinal Tract; Pantoprazole belongs to Gastrointestinal Tract.

Niveau de preuve: Moderate

Pourquoi elle survient

Pharmacodynamic: PPI-induced hypomagnesaemia, often with hypokalaemia, lowers the threshold for domperidone-related QTc prolongation.

Conduite à tenir

Use the lowest effective domperidone dose for the shortest time. Check serum magnesium and potassium, particularly if PPI therapy has lasted more than about 3 months or the patient takes diuretics or digoxin, and correct deficits.
