Résumé
Both are loop diuretics, so combining them gives no therapeutic advantage and adds to the risk of ototoxicity, profound diuresis, and electrolyte depletion. Hearing damage can be permanent and the risk rises with high doses, renal impairment, or rapid infusion.
Ce que signifie cette interaction, en termes simples
Ethacrynic acid and Furosemide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ethacrynic acid belongs to Cardiovascular; Furosemide belongs to Cardiovascular.
Pourquoi elle survient
Duplicate loop diuretic action with additive inhibition of the Na-K-2Cl cotransporter, including in the cochlea, producing additive ototoxicity and volume and electrolyte depletion.
Conduite à tenir
Avoid concurrent use of two loop diuretics; use a single agent and, if resistance occurs, add a thiazide-type diuretic instead. Monitor hearing, renal function, and electrolytes if exposure cannot be avoided.
