Résumé

Fluoxetine inhibits CYP2D6 and CYP3A4 and can raise pimozide concentrations, increasing the risk of QT prolongation and torsades de pointes. Both product labels contraindicate the combination. The effect is dose-related and most dangerous in people with cardiac disease, electrolyte disturbances or CYP2D6 poor metaboliser status.

Ce que signifie cette interaction, en termes simples

Fluoxetine and Pimozide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Fluoxetine belongs to Central Nervous System; Pimozide belongs to Central Nervous System.

Niveau de preuve: High

Pourquoi elle survient

Fluoxetine and norfluoxetine inhibit CYP2D6 and to a lesser extent CYP3A4, reducing pimozide clearance, and both drugs may have additive effects on cardiac repolarisation.

Conduite à tenir

Do not co-administer. Use an alternative antidepressant or antipsychotic without QT or CYP-inhibitory interaction, and obtain an ECG and correct potassium and magnesium if either drug has recently been taken.
