Per cosa si usa
Come agisce
Thyroid hormones act by controlling DNA transcription and protein synthesis. T3 and T4 diffuse into the cell nucleus and bind to thyroid receptor proteins attached to DNA, activating gene transcription and messenger RNA synthesis.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Hypothyroidism: start 25 mcg orally once daily; increase by 25 mcg daily every 1 or 2 weeks if needed; usual maintenance 25 mcg to 75 mcg once daily
Gravidanza e allattamento
- Gravidanza
- Safe Post-marketing data have not shown more birth defects or miscarriage. Untreated hypothyroidism harms mother and fetus, so do not stop treatment: check TSH and adjust the dose during pregnancy. Levothyroxine is the usual first choice in pregnancy.
- Allattamento
- Safe Liothyronine (T3) is a normal component of milk; replacement doses are not a reason to stop breastfeeding (LactMed). US label: weigh benefits (insufficient data on infant effects). Levothyroxine is recommended for hypothyroidism while breastfeeding.
Effetti indesiderati segnalati
- Arrhythmias
- Headache
- Nervousness
- Insomnia
- Tremors
- Weight loss
Avvertenze in evidenza
- Not for treatment of obesity or for weight loss. In euthyroid patients, doses within the range of daily hormonal requirements are ineffective for weight reduction. Larger doses may produce serious or even life-threatening toxicity, particularly with sympathomimetic amines.
Controindicazioni
- Uncorrected adrenal insufficiency
Chi deve prestare attenzione
- Cardiac adverse reactions in the elderly and patients with cardiovascular disease; start at lower doses
- Not for myxedema coma; use intravenous thyroid hormone products
- Acute adrenal crisis in adrenal insufficiency; give glucocorticoids first
