Come agisce
Humanised IgG4 monoclonal antibody that binds the PD-1 receptor and blocks its interaction with PD-L1 and PD-L2, potentiating T-cell responses including anti-tumour responses.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
Extensive-stage SCLC (with carboplatin and etoposide): 4.5 mg/kg bodyweight, IV, every 3 weeks in induction (4 cycles) and maintenance, until disease progression or unacceptable toxicity

- Unsafe No human data. PD-1 blockade causes embryo-fetal toxicity in animals, and IgG4 crosses the placenta. EU label: not recommended in pregnancy or without contraception; use effective contraception during treatment and for at least 6 months after the last dose.
- Allattamento
- Unsafe Not known if it passes into milk. EU label: risk to the infant cannot be excluded in the first days after birth; afterwards it could be used if needed. Other PD-1 antibody labels: avoid for 4 to 5 months after the last dose; nivolumab accumulated in one mother's milk. Avoid during treatment.

Controindicazioni
- Hypersensitivity to the active substance or to any excipient.
Chi deve prestare attenzione
- Immune-mediated adverse reactions, including severe and fatal cases, can occur, up to 3.6 months after the last dose; withhold and give corticosteroids (1-2 mg/kg/day prednisone or equivalent) according to severity.
- Monitor for immune-mediated pneumonitis, colitis, hepatitis (monthly liver tests), nephritis (monthly renal function), endocrinopathies, skin reactions, pancreatitis and myocarditis.
- Infusion-related reactions can occur; monitor patients, reduce rate or interrupt for Grade 2, stop permanently for Grade ≥ 3.
