Per cosa si usa
Come agisce
Selective, orally bioavailable small-molecule inhibitor of BCL-2, an anti-apoptotic protein. It binds BCL-2 directly, displacing pro-apoptotic proteins like BIM, which restores apoptosis through mitochondrial outer membrane permeabilization and caspase activation.
Dose abituale nell'adulto (sintesi del foglio illustrativo)
CLL/SLL: 5-week ramp-up orally once daily (Week 1 20 mg, Week 2 50 mg, Week 3 100 mg, Week 4 200 mg), then 400 mg once daily from Week 5 and beyond
Gravidanza e allattamento
- Gravidanza
- Unsafe May cause fetal harm: in mice it caused embryo loss and lower fetal weight at about 1.2 times human exposure; no malformations; no human data. Avoid; use effective contraception during treatment and for 30 days after the last dose.
- Allattamento
- Unsafe No human milk data; it passes into rat milk. Because of possible serious infant reactions, do not breastfeed during treatment and for 1 week after the last dose. LactMed: most sources advise against breastfeeding during cancer drug therapy.
Effetti indesiderati segnalati
- Neutropenia
- Thrombocytopenia
- Anemia
- Diarrhea
- Nausea
- Fatigue
Controindicazioni
- Concomitant use with strong CYP3A inhibitors at initiation and during the ramp-up phase in patients with CLL/SLL, due to increased TLS risk
Chi deve prestare attenzione
- Tumor lysis syndrome, including fatal events and renal failure requiring dialysis; assess risk, give hydration and anti-hyperuricemics, and monitor blood chemistries
- Neutropenia: monitor complete blood counts throughout treatment and consider supportive measures
- Fatal and serious infections such as pneumonia and sepsis; withhold for Grade 3 and 4 infection until resolution
