Sintesi
Blinatumomab causes a transient cytokine release at treatment initiation and after dose re-starts, which can temporarily suppress CYP450 activity. Cyclosporine is a narrow therapeutic index CYP3A4 substrate, so its blood concentrations may rise and increase the risk of nephrotoxicity and neurotoxicity.
Cosa significa questa interazione in parole semplici
Blinatumomab and Cyclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Blinatumomab belongs to Oncology; Cyclosporin belongs to Miscellaneous.
Perché avviene
Cytokine release (e.g., IL-6) transiently downregulates CYP450 enzymes, reducing cyclosporine metabolism.
Cosa fare
Monitor cyclosporine trough concentrations and renal function closely during the first days of the first cycle and after re-initiation of blinatumomab. Reduce the cyclosporine dose according to levels and clinical signs of toxicity.
Cosa chiedere al farmacista
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Can I take Blinatumomab with Cyclosporin?
MediHelp rates the interaction as moderate. Blinatumomab and Cyclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken. Cyclosporine is a narrow therapeutic index CYP3A4 substrate, so its blood concentrations may rise and increase the risk of nephrotoxicity and neurotoxicity.
How serious is the interaction between Blinatumomab and Cyclosporin?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor cyclosporine trough concentrations and renal function closely during the first days of the first cycle and after re-initiation of blinatumomab. Reduce the cyclosporine dose according to levels and clinical signs of toxicity.
