Sintesi
Both mafenide, applied to burns, and dichlorphenamide inhibit carbonic anhydrase. Mafenide absorbed over large burn areas can cause metabolic acidosis, and adding systemic dichlorphenamide may deepen this and cause hyperventilation.
Cosa significa questa interazione in parole semplici
Dichlorphenamide and Mafenide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dichlorphenamide belongs to Miscellaneous; Mafenide belongs to Dermatology.
Perché avviene
Additive carbonic anhydrase inhibition leading to bicarbonate wasting and metabolic acidosis.
Cosa fare
Monitor arterial blood gases or serum bicarbonate closely, especially with extensive burns. Consider stopping mafenide or holding dichlorphenamide if significant acidosis occurs.
