Nelfinavir is an inhibitor of the HIV-1 protease.

成人常用劑量（仿單摘要）

HIV-1 infection, adults and adolescents 13 years and older (tablets): 1250 mg twice daily or 750 mg three times daily, oral, with a meal, in combination with other antiretrovirals

Unknown Consider alternative antiretrovirals during pregnancy because of hepatic adverse events and decreased exposure in 2nd/3rd trimesters; APR data show a birth defect rate of 3.9% with first trimester exposure. Clinical monitoring is recommended if used.

Unsafe Nelfinavir is present in low levels in human milk; mothers are instructed not to breastfeed because of HIV transmission and infant adverse reaction risks.

  • Coadministration with drugs highly dependent on CYP3A for clearance whose elevated concentrations cause serious or life-threatening events (e.g. alfuzosin, amiodarone, quinidine, lurasidone, pimozide, ergot derivatives, cisapride, lovastatin, simvastatin, sildenafil for PAH, triazolam, oral midazol…
  • Rifampin and St. John's wort are contraindicated because they reduce nelfinavir concentrations, risking loss of effect and resistance.

  • Drug interactions via CYP3A may cause serious adverse reactions or loss of efficacy; review concomitant medications.
  • Should not be used in moderate or severe hepatic impairment.
  • Oral powder contains phenylalanine (11.2 mg per gram), harmful in phenylketonuria.
