Summary

Combining a cephalosporin with an aminoglycoside may raise the risk of kidney injury, and amikacin itself is nephrotoxic and ototoxic. Oral cefdinir is not notably nephrotoxic, so the added risk is small and mostly relevant in patients with renal impairment or other risk factors.

What this interaction means in plain words

Amikacin and Cefdinir have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Possible additive nephrotoxicity, though not well established for oral cefdinir.

What to do

Monitor serum creatinine and amikacin levels as is routine for aminoglycoside therapy, and adjust the dose to renal function.
