Summary

NSAIDs reduce renal prostaglandin-mediated perfusion and can lower glomerular filtration, which may raise aminoglycoside concentrations. This has been described mainly in neonates, but renal compromise from NSAIDs can also increase aminoglycoside nephrotoxicity in adults.

What this interaction means in plain words

Amikacin and Lornoxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amikacin belongs to Anti Bacterial; Lornoxicam belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

NSAID inhibition of renal prostaglandins reduces renal blood flow and aminoglycoside clearance.

What to do

Monitor amikacin trough levels and serum creatinine, especially in neonates, the elderly or dehydrated patients. Use a non-NSAID analgesic where possible.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
